Service Description
Course Description — Targeted Case Management: Substance Use Disorders (5 hours live instruction, 1 hrs on-demand video) This course teaches Kentucky Targeted Case Managers how to support people with substance use disorders from first contact through discharge. In clear, practical steps, learners review how addiction and recovery work, how to match services to ASAM levels of care, how to use Kentucky and federal resources (benefits, treatment locators, recovery supports), how to run effective team meetings, and how to document care plans, discharge/aftercare, and relapse-prevention plans that meet Medicaid and DBHDID expectations. The class uses short skills labs, checklists, and Kentucky-specific links so participants leave with tools they can use the same day.
