About
Course Description — Targeted Case Management: Co-Occurring Behavioral Health & Chronic Physical Conditions (6 hours) This course prepares Targeted Case Managers to support adults and youth who have both a behavioral health condition (SMI/SED/SUD) and a chronic or complex physical health condition. Using Kentucky-specific expectations, learners practice plain-language assessment, co-occurring care planning, medication/side-effect awareness (non-prescribing role), barrier-busting for access and adherence, and coordinated linkage to medical, behavioral, and community resources. The class includes brief skills labs, local resource mapping, and checklists aligned to the rubric so documentation and service delivery meet review standards.
