Course Description — Targeted Case Management: Children & Youth with SED This course prepares Targeted Case Managers to support children and youth with Severe Emotional Disability (SED) and their families using Kentucky’s wraparound approach. Learners practice engagement, needs/strengths assessment, care-planning, meeting facilitation, monitoring, and transition—always centering family/youth voice, natural supports, culture, and real-world outcomes. The class follows the four wraparound phases and the ten wraparound principles, and includes short skills labs, sample questions, and ready-to-use checklists that line up with the DBHDID rubric.
